Change of address, name, insurance information, and medications

Change of Insurance/Medicare:
If you have changed insurance providers or switched to Medicare, please bring a copy of your new card with you. NOTE: If you have recently switched to Medicare, you will be required to complete paperwork to update your file and a detailed exam including range of motion and current injury.
