*Late Night Hours: We are open until 5:30 PM every Thursday.

Holiday Hours: We are open every weekday holiday until 10:00 AM, except Christmas Day (closed).

Change of Insurance/Medicare:
If you have changed insurance providers or switched to Medicare, please bring a copy of your new card with you. NOTE: If you have recently switched to Medicare, you will be required to complete paperwork to update file and a range-of-motion exam.
